Those two are the only ones that will actually do anything. Changing the cl_maxfps only is noticable if it's already set below 50 or something.

what is your fps?

500?

turn off vsnc and check to make sure your anisotropic settings is 2x on card. Helps me at least.

Caustics and stainmaps will do almost nothing, but here they are anyway.

All you need to do is delete your hard drive, then soak it in hot water for 45 minutes.  After pissing all over it and reassembling it, the framerate will skyrocket.  You're welcome.
